Linda is a single mother living with late-stage kidney failure, requiring dialysis three times a week.

She cares for her four-year-old son, John, who has complex developmental and behavioural needs. John requires close supervision and thrives in a stable, familiar environment.

When Elderfolks became involved, it was clear the family was under significant pressure. Their existing housing was not suitable to support Linda’s medical treatment or provide the consistency John needed.

 

The Challenge

Linda needed urgent access to a home that could safely support dialysis while allowing her to continue caring for her son.

Without appropriate housing, her health was at risk—and in turn, her ability to provide consistent care and supervision for John was becoming increasingly difficult. The situation required a solution that addressed both medical and family needs together.

 

Our Approach

Elderfolks coordinated a comprehensive functional capacity and home environment assessment with our Occupational Therapy team to support an urgent housing transfer.

This assessment provided clear clinical evidence of Linda’s medical requirements and the environmental supports needed for both her and John.

At the same time, we worked collaboratively with an NDIS provider to initiate early intervention and behavioural supports for John—ensuring he had the right structure and care in place while Linda focused on managing her health.

 

The Outcome

With strong clinical evidence and targeted advocacy, Linda was supported to apply for an urgent housing transfer to a more suitable home.

In her new environment, Linda is able to safely manage her dialysis at home, while John benefits from greater stability and access to appropriate supports.

Together, they now have a home that better supports their health, wellbeing, and daily life.
