Margaret is a 72-year-old woman living with an intellectual disability, significant cognitive challenges, and complex health needs.

For approximately 18 months, Margaret experienced homelessness, often sleeping at a busy train station. Prior to this, she had been living in extreme hoarding and squalid conditions, likely connected to her cognitive impairment. Over time, her health and safety became increasingly at risk.

 

The Challenge

Margaret’s situation required a thoughtful and sensitive approach.

Already under the Public Guardian, decisions around her care and living arrangements needed to be made collaboratively, ensuring her best interests were protected while still respecting her voice and preferences.

Our team needed to balance supporting Margaret’s participation in decisions with ensuring she could access the level of care and safety her situation required.

 

Our Approach

We began by building trust.

Our Homelessness Team met Margaret where she felt most comfortable, taking the time to develop a consistent and respectful connection. Through patience and understanding, Margaret began to feel heard and supported.

Working closely with the Public Guardian and the Homeless Health Service, we took a collaborative approach to understanding Margaret’s full story. Together, we built a comprehensive picture of her medical history, family background, personal preferences, and what mattered most to her day-to-day. This shared understanding guided the search for an aged care home that could genuinely meet her complex needs, somewhere safe, supportive, and aligned with who she is.

 

The Outcome

Margaret transitioned into a caring and secure aged care home. A place for her to call home, where she has made friends and the staff have become her family.

While the change was initially challenging, she gradually became more comfortable as her new environment became familiar. For the first time in years, Margaret experienced stability, routine, and consistent support.

With the ongoing involvement of the Public Guardian, her accommodation and care arrangements are now safeguarded, providing long-term security. Today, Margaret is safe, supported, and living with dignity.

 

Why It Matters

Margaret’s story reflects the importance of collaboration, compassion, and person-centred support, especially in complex situations.

By working alongside the Public Guardian and the Homeless Health Service and taking the time to build trust with Margaret, we were able to support her transition from homelessness to a stable and supportive home, where she could age with the dignity, care and respect she deserves.

It’s a reminder that with the right partnerships and a respectful approach, even the most complex circumstances can lead to positive, life-changing outcomes.
